State Issued Driver's License
State Issued ID Card
Canada Driver's License
US Passport
US Passport Card
Foreign Passports stamped by USCI
Permanent Resident Alien Card (Green Card)
US Military ID Card
Government Issued Employment ID
Some Employment ID's with a picture are acceptable
If no such ID is available and cannot be reasonably obtained, the signer may have TWO CREDIBLE WITNESSES, WITH THEIR OWN VALID IDENTIFICATION, attest UNDER OATH as to their identity.
